This position assists in development, implementation, revision and reporting of case management program and is accountable for coordinating clinical and financial outcomes of an assigned population. He/She works collaboratively with all interdisciplinary staff to achieve timely, cost efficient and effective management of patient care at HomeReach. Ensures specific patient outcomes are achieved.

EDUCATION and/or EXPERIENCE
Associates Degree in Nursing and RN license in the State of Ohio.

Assists patients and their families with social, economic and psychological needs which may affect their abilities to benefit fully form health care. Screens, identifies, assesses, develops and implements a plan of care for patients and families requiring social work or discharge planning.

EDUCATION: Masters degree in Social Work or a related field and LSW license in the State of Ohio. Knowledge of human growth and development and psychosicial interventions. Preferred 2-3 years hospital experience in LSW and knowledge of medical terminology and computer skills.

SPECIALIZED KNOWLEDGE: Possess organizational and analytic skills, which enables this position to function independently. Demonstrates effective verbal, written, and effective interpersonal skills. Must utilize social work theory, skills and interventions in a health care setting. Familiar with basic medical terminology and basic computer skills.

Monitors adherence to the hospital’s utilization review plan to ensure effective and efficient use of hospital services and reviews the appropriateness of hospital admissions and extended hospitals stays. He/She works collaboratively with all interdisciplinary staff internal to OhioHealth and also external organizations to achieve timely, cost efficient and effective management of patient care. Performs utilization review in accordance with all state and federal mandated regulations. Maintains compliancy with regulation changes affecting utilization management.

MINIMUM QUALIFICATIONS
Graduate from an accredited Bachelor's of Nursing program; Current Ohio RN license. and/or Graduate from an accredited Master's of Social Work Program and or a LISW license. . Specialized knowledge of discharge process, utilization management intensity of service, severity of illness criteria; Familiarity with computer technology, and ability to use database and spreadsheets; Excellent written/verbal communication and teaching/training skills; Demonstrated success in achieving or exceeding measurable performance criteria in a job setting.

DESIRABLE QUALIFICATIONS
Advanced training in case management, or certification in case management or clinical specialty. Master's Degree in related field. Case management and/or performance improvement experience. Experience in financial analysis. Experience in group/collaborative teamwork process. Experience in teaching.

Looking for medical social worker to work contingent hours on weekdays and weekends on all areas of hospital, including med-surg and OB. Hospital experience preferred.
