Position Summary
This position assists in development, implementation, revision and reporting of case management program and is accountable for coordinating clinical and financial outcomes of an assigned population. He/She works collaboratively with all interdisciplinary staff to achieve timely, cost efficient and effective management of patient care at HomeReach. Ensures specific patient outcomes are achieved.
Minimum Qualifications
EDUCATION and/or EXPERIENCE
Associates Degree in Nursing and RN license in the State of Ohio.
SPECIALIZED KNOWLEDGE
Acute care experience
KIND and LENGTH of EXPERIENCE
1-2 yrs related experience.
