Position Summary

Coordinates provision of discharge planning that reflects the hospital's philosophy and standards for continuity of care, to ensure quality and safe delivery of discharge planning.
To assist through nursing assessment a patient's continuity of care needs at admission, during stay, and including post discharge services. Accomplishments would be a team effort, better patient health outcomes, reduced length of stay for non-medical reasons and reduced admission rates. To assist with planning, implementation and evaluation of the discharge planning program throughout the hospital.

Minimum Qualifications

Baccalaureate prepared. Ohio license. Knowledge of community health and nursing, and community resources. Knowledge and skill in clinical assessment. Knowledge of rehab, principles and practices. Five years recent practice with one year experience in public health nursing or related community agencies.

Additional Job Information

Contingent case manager position at Grant Medical Center. Must have RN license and a Bachelors degree.
